Medical Weight Loss · Physician-Supervised

Weight loss isn't a willpower
problem. It's a hormone problem.

Our GLP-1 program uses semaglutide and tirzepatide — the same molecules behind Wegovy and Zepbound — sourced through a licensed Texas 503A compounding pharmacy and titrated in-clinic by a physician. No mail-order. No algorithms. Real medicine.

The Real Story

Your body has been fighting you — and winning.

Diet and exercise fail most people not because of laziness — but because the biology of obesity is designed to defend stored fat. Until you address the hormones, you cannot win.

The hunger drive is hormonal.

Ghrelin, leptin, and GLP-1 dictate when you feel hungry and when you feel full. In people with obesity, these signals are blunted — your brain receives "feed me" messages long after you have eaten enough. That is biology, not weakness.

Your body defends a set-point.

After weight loss, metabolism slows and hunger hormones spike to push you back to the original weight. This is why 80% of dieters regain within five years. Medication resets the set-point your hypothalamus is defending.

Insulin resistance keeps the cycle going.

Excess weight drives insulin resistance, which drives fat storage, which drives more hunger. GLP-1 agonists improve insulin sensitivity and break the feedback loop at the receptor level — not at the willpower level.

"Obesity is a chronic, relapsing, hormonal disease. Treating it with shame instead of medicine has failed an entire generation."
How It Works

Simple, supervised, and done right.

Three deliberate steps. No mail-order syringes, no chatbots, no guesswork on dosing — just the standard of care obesity medicine deserves.

01

Initial assessment & labs

One in-person visit with Dr. Sani. Full history, medication review, weight-related health screen, and baseline labs (A1c, lipid panel, kidney function, TSH). We confirm candidacy and rule out contraindications.

02

Personalized GLP-1 protocol

Semaglutide or tirzepatide — whichever fits your physiology, goals, and budget. We prescribe through a licensed Texas 503A compounding pharmacy and set a slow, conservative titration schedule built around your tolerance.

03

Weekly visits + titration

Every week, a nurse administers your injection in-clinic. Vitals, side-effect check, and dose adjustments happen in real time. Monthly provider visits review progress, labs, and the long-term plan.

What our weight loss patients say

Real first names. Real Texoma neighbors. Real numbers on the scale — no stock photos, no paid actors.

Lost 35 lbs
Maria T.
Sherman, TX
I was skeptical about another weight loss program. The difference here is they actually see you every week. My A1c dropped from 6.4 to 5.6 and I'm off two of my old medications.
Down 42 lbs
Robert H.
Denison, TX
The slow titration made all the difference. No nausea, no crashes — just steady progress. Knowing a physician was overseeing my dose changes gave me peace of mind.

Evidence-Based Care

Built on the strongest obesity-medicine evidence of the last decade.

Our protocols mirror the dosing schedules used in the NEJM STEP-1 trial for semaglutide and the SURMOUNT-1 trial for tirzepatide — both showing 15-22% total body weight loss over 68-72 weeks. We monitor patients per the NIDDK guidelines for prescription anti-obesity medications, including baseline labs, contraindication screening, and quarterly metabolic reassessment.

Common Questions

What patients ask before starting

If your question isn't covered here, call us — we'd rather answer it before you book than guess at it later.

Am I a candidate for the GLP-1 program?
Most adults with a BMI of 27 or higher (or 25+ with weight-related conditions like prediabetes, hypertension, or sleep apnea) are eligible. We screen for contraindications such as personal or family history of medullary thyroid carcinoma, MEN-2 syndrome, active pancreatitis, or pregnancy. Candidacy is determined during your initial in-person evaluation with labs.
What side effects should I expect?
The most common side effects are gastrointestinal — nausea, mild reflux, constipation, or transient diarrhea — usually during the first week of each dose increase. These typically resolve as your body adapts. Slow, supervised titration is the single biggest factor in tolerability, which is exactly why we titrate in-clinic rather than mailing you the maximum dose.
Is the program covered by insurance, or is it cash-pay?
The compounded GLP-1 medication itself is offered as a transparent cash-pay program starting at $99/month. Your monthly provider visit and any associated labs (A1c, lipid panel, kidney function, thyroid) are billed to your insurance under the obesity or metabolic-disease diagnosis. We will verify your benefits before your first visit.
What's the difference between branded and compounded semaglutide/tirzepatide?
Branded versions (Wegovy, Ozempic, Zepbound, Mounjaro) require commercial insurance approval and typically cost $1,000-$1,300/month out of pocket without coverage. We use 503A-compounded semaglutide and tirzepatide from a licensed Texas compounding pharmacy — same active molecule, regulated under USP 797/795 sterile compounding standards, at a fraction of the cost. We will discuss both pathways at your consult.
What happens if I stop the medication?
GLP-1 therapy is best understood as a long-term treatment for a chronic condition, similar to medication for hypertension or cholesterol. When the medication is stopped, appetite signaling returns to its previous state and weight regain is common without a structured maintenance plan. We work with each patient on a long-term strategy — including step-down protocols, nutrition support, and resistance training — to preserve results.
Won't I just gain the weight back?
Studies (STEP-4, SURMOUNT-4) show that abrupt discontinuation leads to partial regain, but patients on long-term maintenance dosing — combined with adequate protein intake, strength training, and continued medical follow-up — preserve the majority of their weight loss. Our program is built around that long-term framework, not a 12-week sprint.
